Plymouth is one of the fastest growing suburbs in Sheboygan County. Our program site is easily accessible, located just south of Highway 23 at the corner of Route 57 and County Road C. This location offers accessible space with several breakout rooms for individuals with developmental disabilities and brain injuries. Some program areas are evolving and will be established based on the needs of the individuals we serve.

• Ambulation Support (i.e. wheelchairs, walkers, canes)
• Incontinence Care (i.e. depends, toileting schedules)

• Special Feeding Assistance (i.e., g-tube, one on one feeding, choking prevention)
• Seizure Support (Vagus Nerve Stimulators, PRN protocols)
• Other Conditions Requiring Daily Medication Administration
