This site is centrally located in Green Bay, providing many excellent opportunities for off-site activities and community involvement. In fact, this site boasts five to six community outings per day! The program currently supports adults with developmental disabilities with significant behavioral support needs in a two-story open floor plan. Social Integration skills and community involvement are a major focus at this site.

On selected Saturdays, the site supports children and young adults, ages 8-21 in a respite program (Paragon Activity Club). The purpose of the program is to provide families with a respite alternative that encourages recreation and socialization opportunities for their children with disabilities.

Health and Personal Needs Services include but are not limited to

• Ambulation Support (i.e. wheelchairs, walkers, canes)
• Incontinence Care (i.e. depends, toileting schedules)
• Catheterization
• Diabetes Care
• Special Feeding Assistance (i.e., g-tube, one on one feeding, choking prevention)
• Seizure Support (Vagus Nerve Stimulators, PRN protocols)
• Other Conditions Requiring Daily Medication Administration
