Paragon's Fort Atkinson facility is located on the west side of Fort Atkinson just off Highway 26. The program has been in operation since 1996 and recently expanded in 2005. All three adult program areas – Brain Injury, Sensory Experience and Social Integration – are offered at this site. Each program has a separate entrance and activity rooms appropriate for participants' interests and levels of care. There is also a fenced in outdoor area, basketball hoop and lots of yard space for picnicking, gardening and other outdoor activities.

Health and Personal Needs Services include but are not limited to

  • Ambulation Support (i.e. wheelchairs, walkers, canes)
  • Incontinence Care (i.e. depends, toileting schedules)
  • Catheterization
  • Diabetes Care
  • Special Feeding Assistance (i.e., g-tube, one on one feeding, choking prevention)
  • Seizure Support (Vagus Nerve Stimulators, PRN protocols)
  • Other Conditions Requiring Daily Medication Administration
